Beneficiary characteristics were identified from the Medicare enrollment database, which includes information on race/ethnicity from the Social Security Administration and the RTI recoded race variable that uses surname analysis to improve identification of Hispanic ethnicity; dual-enrollment in Medicaid.
Beneficiary residence is based on the location of the beneficiary’s home in an urban area based on Metropolitan Statistical Area (MSA) or rural if not in a MSA.
**Part B clinician-billed services as “visits,” as this mostly includes doctor’s offices, clinics, and hospital outpatient department.
