Supplementary notes

Measured using the score from the Patient Health Questionnaire (PHQ-9), a nine item depression-screening instrument that asks about the frequency of symptoms of depression in the past 2 weeks. Response categories of “not at all,” “several days,” “more than half the days,” and “nearly every day” are given a score of 0 to 3. Summary scores ranged from 0 to 27. Depression was defined using a score of 10 or higher, a well-validated cut point used in primary care settings.
