There are three different types of SNPs:
Chronic Condition SNP (C-SNP) - for people with disabling chronic condition, such as End Stage Renal Disease (ESRD), severe diabetes, dementia, or cancer.
Dual Eligible SNP (D-SNP) - for people who are eligible for coverage under both Medicare and Medicaid.
Institutional SNP (I-SNP) - for people residing in a long-term care facility, or living at home but requiring an institutional level of care.
