JQ number of reported near miss medical errors in Japan 2021, by incident type
In 2021, approximately ten thousand cases of medical near misses related to medicines were reported by medical facilities to Japan Council for Quality Health Care (JQ). Near misses refer to mistakes that were almost made or actually made but got found before they resulted in harm of the patients. That year, a total of close to 29.8 thousand medical near misses were reported to JQ. Designated by the health ministry in the country, JQ conducts various third-party evaluations of healthcare institutions and provides a database of medical adverse events and near miss events with the aim of improving the healthcare system in Japan.
