Japan: number of medical adverse events by report type 2022

In 2022, a total of 5,313 medical errors were reported to the Japan Council for Quality Health Care (JQ). 4,631 cases of which were submitted by medical facilities that were registered in JQ's database and subject to reporting requirements. 682 cases were from medical facilities which voluntarily submitted the reports.
Designated by the health ministry in the country, JQ conducts various third-party evaluations of healthcare institutions and provides a database of medical adverse events and near-miss events with the aim of improving the healthcare system in Japan.
